California law requires
The driver of a vehicle involved in an accident resulting in an injury, death or property damage in excess of $1,000 must submit a SR-1 Form to the California Department of Motor Vehicles within 10 days.

Failure to comply may result in suspension of your driver's license. Law enforcement reports do not satisfy the DMV report requirement. 

You can get a Form SR-1 from:

  • A Department of Motor Vehicles office
  • Any police station
  • A motor vehicle club
  • Insurance agency
  • DMV website

Once you are finished filing your report, keep your report as it will be your record of the accident.
